障害対応の振り返り

月曜日の午後、本番環境で大規模な障害が発生した。原因は新機能に関する設定ミスであり、サービスは約二時間にわたって停止した。

げつようび の ごご、ほんばん かんきょう で だいきぼな しょうがい が はっせい した。げんいん は しんきのう に かんする せってい みす で あり、さーびす は やく にじかん に わたって ていし した。

On Monday afternoon, a large-scale failure occurred in the production environment. The cause was a configuration mistake related to the new feature, and the service was down for about two hours.

障害発生直後、担当チームは直ちに対応を開始した。まず影響範囲を特定した上で、設定を以前の状態に復元し、サービスを復旧させた。復元の作業自体は約三十分で終わった。障害は当初、担当者のミスとして報告されていた。

しょうがい はっせい ちょくご、たんとう ちーむ は ただちに たいおう を かいし した。まず えいきょう はんい を とくてい した うえ で、せってい を いぜん の じょうたい に ふくげん し、さーびす を ふっきゅう させた。ふくげん の さぎょう じたい は やく さんじゅっぷん で おわった。しょうがい は とうしょ、たんとうしゃ の みす として ほうこく されて いた。

Immediately after the failure, the team in charge began responding. After identifying the scope of impact, they restored the configuration to its previous state and recovered the service. The restore work itself finished in about thirty minutes. At first the failure had been reported as an individual engineer's mistake.

振り返りの会議では、個人を責めることなく、仕組みの問題として原因を分析した。レビューの工程が形だけになっていたこと、テスト環境と本番環境の差異が大きかったことが指摘された。今後は設定の変更を二人で確認することとし、改善の結果は火曜日の会議で報告することになった。

ふりかえり の かいぎ で は、こじん を せめる こと なく、しくみ の もんだい として げんいん を ぶんせき した。れびゅー の こうてい が かたち だけ に なって いた こと、てすと かんきょう と ほんばん かんきょう の さい が おおきかった こと が してき された。こんご は せってい の へんこう を ふたり で かくにん する こと と し、かいぜん の けっか は かようび の かいぎ で ほうこく する こと に なった。

In the retrospective meeting, the cause was analyzed as a process problem, without blaming individuals. It was pointed out that the review step had become a formality and that the gap between the test and production environments was large. From now on, configuration changes will be checked by two people, and the results of the improvements will be reported at the Tuesday meeting.

失敗をかくすのではなく、組織の学びに変える。この文化こそが、システムの信頼性を支える基盤であると改めて感じた。
